Liability Waiver

By signing this waiver, I acknowledge and agree to the following terms and conditions:

I, the undersigned, am the parent or legal guardian of the child(ren) listed below (the “Child” or “Children”) and I am fully aware of the potential risks involved in allowing my child(ren) to participate in activities at Playroom LLC, DBA Kidsville Play Cafe (the “Facility”).

I acknowledge that, despite efforts to maintain a safe environment, accidents can occur. I accept full responsibility for any injuries, damages, or losses that may occur to my child(ren) while participating in activities at the Facility, including but not limited to injuries resulting from:

Slipping, falling, or tripping on the play area or other surfaces. Injuries caused by play equipment, toys, or materials used in activities. Contact or physical interaction with other children, which may lead to accidental injuries such as pushing, falling, hitting, or fighting. Choking hazards, including choking on food snacks, toys, or other small objects that may be accessible during play.

Physical play with toys or equipment. Slipping or falling on surfaces, including but not limited to floors, carpets, or mats. Choking on food, snacks, or small toys that may be present in the play area.

I agree that Kidsville Play Cafe and its staff are not liable for any injury, illness, or loss sustained by my child(ren) while attending or using the services or equipment at the Facility. I understand that accidents can happen even in a supervised environment, and I take full responsibility for the safety of my child(ren) during their time at the Facility.

In the event of an emergency, I give permission for the staff at Kidsville Play Cafe to seek medical attention for my child(ren) if deemed necessary. I agree to pay any medical expenses incurred as a result of an injury sustained by my child(ren) while attending the Facility.

I agree to comply with all rules and guidelines provided by Kidsville Play Cafe, including supervising my child(ren) at all times while in the play area.

By signing this waiver, I agree that Kidsville Play Cafe and its affiliates, employees, officers, directors, agents, and volunteers are not responsible for any injury, illness, or loss, and I release them from any and all claims related to my child(ren)’s participation in activities at the Facility.
